Paul Merrick, MD, Chief Physician Executive and Chairman, Duly Health andCare
Dr. Paul Merrick, aboard-certified and renowned urologist, serves as the Chair of the Clinical Board, Co-Chairman of the Duly Health and Care Board and Chief Physician Executive. In his roles, Dr. Merrick leads the Board and physicians in their goal of making healthcare better for patients and communities by offering high-quality and compassionate care. He ensures that Duly’s physician-driven approach continues to lead to exceptional physician engagement and improved patient outcomes.
He has been in practice since 1994, was afounding physician when Duly Health and Care started as DuPage Medical Group in 1999 and has been leading the physician organization since 2009. He previously served as chairman of the Urology department for 13years; he also previously served as President and Treasurer of the Clinical Board. Dr. Merrick earned his medical degree from Rush Medical College and completed his residency at Rush University Medical Center, where he served as chief resident of urology.

Christina E. Bratis, MD, President, ClinicalBoard
Dr. Christina Bratis, ahospitalist board-certified in internal medicine, serves as President of the Clinical Board of the Duly Health and Care physician organization. Dr. Bratis joined Duly Health and Care in August 2009 and has served on the Clinical Board of Directors since 2017. She has held many other leadership roles including that of Hospitalist Department Chair from 2014 – 2018 and Chair of several Board Committees.
Within the community, Dr. Bratis played an instrumental role as the Clinical Chief of Staff of the McCormick Place Alternate Care Facility, the first local field facility built to address the city of Chicago and state of Illinois’ COVID-19 response.
She earned her medical degree from Rush Medical College, completed her residency at the University of Illinois at Chicago, and served an additional year there as chief resident in internal medicine. She is affiliated with Advocate Good Samaritan Hospital, Edward Hospital, Elmhurst Hospital and Northwestern Medicine Central DuPage Hospital.

Abdul Sankari, MD, MBA, FACC, FCCP, Vice President, ClinicalBoard
Dr. Abdul Sankari, board certified in Interventional Cardiology, Nuclear Cardiology, Critical Care
Medicine, Cardiovascular Disease, and Internal Medicine, serves on the Clinical Board of the Duly Health and Care physician organization. He joined Duly Health and Care in 2016 after serving nearly 15years as apracticing physician with Heartland Cardiovascular where he held numerous leadership roles, including group president.
He received his Medical Degree from the University of Aleppo, Syria, where he earned the First Rank in the Graduation Exam at the University of Aleppo. Dr. Sankari completed his internship and his residency at the University of Illinois, Michael Reese Hospital and Medical Center in Chicago, where he was the Chief Medical Resident. He also completed afellowship in Cardiology at the University of Illinois at Chicago and the Michael Reese Hospital and Medical Center, where he was the Chief Cardiology Fellow. Dr. Sankari completed aCritical Care Fellowship at Emory University.
Dr. Sankari serves as the Director of the Cardiac Catheterization Lab at Silver Cross Hospital, New Lenox. He is the past President of the Heartland Education and Research Foundation. His research has been published in numerous peer-reviewed journals.

Mia A. Taormina, DO, FACOI, Secretary, ClinicalBoard
Dr. Mia Taormina, board-certified in Infectious Disease and Internal Medicine with Hospital Medicine focus, serves on the Clinical Board of the Duly Health and Care physician organization. Dr. Taormina has been apart of Duly Health and Care since 2012 and has served in various leadership roles including as Chair of the Department of Infectious Disease, and her leadership has been highlighted throughout the organization’s COVID response.
Detroit area born and raised, Dr. Taormina graduated from Michigan State University — College of Osteopathic Medicine with honors and served as President of the Student Government. She completed an Internal Medicine residency and Infectious Disease Fellowship at Beaumont Botsford Hospital in Farmington Hills, MI where her leadership skills were further utilized in her roles as Chief Intern and Chief Resident during her training. Dr. Taormina recently received the honor of giving the Keynote Address at her alma mater at the convocation of osteopathic medical school graduates at Michigan State University in2022.

Stefan Nemeth, MD, Treasurer, ClinicalBoard
Dr. Stefan Nemeth, aboard-certified family medicine doctor, serves on the Clinical Board of the Duly Health and Care physician organization. Dr. Nemeth has been in practice for 24years and focuses his practice on patients ages nine and older, with clinical interests in sports injuries, diabetes, hypertension and arthritis.
Dr. Nemeth was the Chief of Medical Staff at Provena St. Joseph Medical Center in 2010. He served as President of Meridian Medical at the time that the group joined DuPage Medical Group in 2015. He has helped continue to grow services in Joliet and clinics serving the Will County area. He earned his medical degree from Loyola University Stritch School of Medicine in Maywood and completed his residency at Hinsdale Hospital. He is affiliated with Amita Health Presence Saint Joseph Hospital in Joliet and Silver Cross Hospital in NewLenox.

David Dungan, MD, ClinicalBoard
Dr. David Dungan, board-certified in internal medicine, pediatrics, and culinary medicine, serves as Secretary of the Clinical Board of the Duly Health and Care physician organization. Dr. Dungan is an experienced internist and pediatrician who specializes in high quality personalized healthcare for allages.
Dr. Dungan joined Duly in 1999 as one of DuPage Medical Group’s founding physicians; outside of his medical practice, he chairs the Quality Assurance and Utilization Management Committee of Duly Health and Care. In addition, he serves on the board of the DuPage Health Coalition representing Duly Health and Care, is newly assigned to the Clinical Practice Committee for the Culinary Medicine Specialist Board, and is aformer board chair for FORWARD (Finding Opportunities that Result in Wellness for All Residents of DuPage), a501c3 charitable organization which was dedicated to building community-based systems to counter the growing problem of chronic disease and health disparities. Dr. Dungan is also aFellow of the American Academy of Pediatrics and aFellow of the American College of Physicians.
He received his medical degree from Loyola University Stritch School of Medicine in Maywood. Dr. Dungan completed his combined residency in internal medicine and pediatrics at Loyola University Medical Center/Foster McGaw Hospital. He is affiliated with Advocate Good Samaritan Hospital.

Thomas Gallagher, MD, ClinicalBoard
Dr. Thomas Gallagher, aboard-certified internal medicine physician, serves on the Clinical Board of the Duly Health and Care physician organization. He was one of the founding members of DuPage Medical Group in 1999 and has practiced in the Hinsdale area throughout his career which began in 1992. Dr. Gallagher has been asite leader at the Hinsdale Clay Street location since 2009. He was also the department of medicine chairman from 2014 until 2017. He has served on numerous committees over the years, including the EMR and Charitable Fund committees. In 2017, Dr. Gallagher stepped away from leadership roles and pursued amaster’s degree in clinical informatics at Northwestern. He completed the degree in 2020 and became board-certified in informatics in2021.
Dr. Gallagher graduated from the University of Michigan and then attended the University of Chicago for medical school. After finishing aresidency at Northwestern, he joined asmall, four physician medical practice in Hinsdale. These physicians ultimately became founding members of DuPage MedicalGroup.

Dr. Hashmi graduated from M.R. Medical College in India, completed her residency in Primary Care Internal Medicine at Cook County Hospital and Rush Presbyterian St. Luke’s Medical Center in Chicago. She joined aprivate practice in Naperville and contributed to its successful growth; leading to an opportunity to merge with DuPage Medical Group in 2008. Since 2008 she has served in various primary care leadership roles within the organization. She is amember of the American Medical Association and the American College of Physicians and affiliated with Edward Hospital.

Jihad Kudsi, MD, MBA, MSF, ABOM, FACS, FASMBS, ClinicalBoard
Dr. Jihad Kudsi, board-certified in surgery and obesity medicine, serves on the Clinical Board of the Duly Health and Care physician organization. Dr. Kudsi has been in practice for 8years, and focuses his practice on holistic care for those patients struggling with obesity and other health conditions requiring surgery. He serves as the Bariatric Surgery Medical Director at the University of Chicago LaGrange Hospital, where his leadership helps shape the course of surgical advancements and patient care. He serves as Chair of the Surgery Department at Duly Health and Care, where he works to advance the group’s surgical practices.
Dr. Kudsi earned his MBA and Master of Science in Finance from Indiana University’s Kelley School of Business in 2020. He received his medical degree from Aleppo University in Syria. He completed his residency in general surgery at George Washington University and fellowship training in minimally invasive bariatric surgery at the Houston Methodist Hospital, Texas Medical Center.

Dan Greenleaf, Chief Executive Officer
Dan Greenleaf serves as Chief Executive Officer at Duly Health and Care. He works in close partnership with leadership and clinicians on advancing Duly’s mission of delivering exceptional clinical excellence and patient care while driving sustainable growth for the independent physiciangroup.
Dan brings nearly 30years of experience leading healthcare services organizations. Most recently he served as President and CEO of Modivcare, which provides aplatform of integrated supportive care solutions focused on improving health outcomes. Prior to that he served as President and CEO of BioScrip, Inc., the largest independent provider of infusion and home care management solutions, where he led the merger with Option Care Health. He also served as CEO of Home Solutions Infusion Services and Coram Specialty Services.
Dan graduated from Denison University with aBachelor of Arts in Economics and holds aMaster of Business Administration in Health Administration from the University of Miami. He spent six years in the United States Air Force where he served as an officer and navigator and did atour in the first GulfWar.

Carol Brockmiller, CEO, Quincy MedicalGroup
Carol Brockmiller serves as the Chief Executive Officer of Quincy Medical Group, which has been part of the Duly Health and Care brand since fall 2021. Carol has been part of QMG for 25years, serving as its CEO since November 2014. Carol is focused on transforming healthcare for residents of Central Illinois and the tri-states of Illinois, Iowa and Missouri and advancing the organization’s culture to provide an unrivaled environment for physicians to deliver healthcare, team members to build life-long careers and patients to experience unparalleledcare.
Carol earned her Bachelor’s degree in Political Science from Quincy University.

Ryan West, President, Duly Health and Care, Chicagoland Operations
Ryan West serves as the President, Chicagoland Operations for Duly Health and Care. In this role he partners with the physicians and leaders throughout the organization to ensure delivery of an exceptional patient experience. Ryan joined Duly in 2021 with more than 15years of healthcare experience, serving in roles across consulting, strategic planning, population health and operations. Prior to joining Duly, Ryan served in several progressive leadership roles at VillageMD, Ascension Health, and SSM Health Care in St. Louis, MO. Most recently, Ryan served as CEO of Duly’s operation in Indiana under The South Bend Clinicbrand.
Ryan is agraduate of Elon University in North Carolina and earned his Master of Health Services Administration from Xavier University.

Kerri Balbone, Chief Revenue Officer
Kerri Balbone serves as Chief Revenue Officer for Duly Health and Care. She is aveteran healthcare leader with more than 30years of experience. In her role, Kerri has oversight for our revenue cycle management functions and payor contracting and partnerships. Kerri has astrong value-based care background that will serve Duly well as we strengthen the VBC care model that Duly is known for delivering to patients.
Prior to joining Duly, Kerri served as the Chief Operating Officer of Health Net, asubsidiary of Centene, where she led the operations for several of Health Net’s businesses representing three million California members. Before joining Health Net, she worked at UnitedHealthcare where she excelled in various leadership roles over her 16-year tenure, ranging from Business Integration, Network &Provider Strategy and Health Plan Operations. She is uniquely versed in understanding our business from apayor perspective and is astrong patient and physician advocate.
Kerri earned her Bachelor of Science degree in Health Systems Management from the University of Connecticut and her MBA in Health Administration from Sacred Heart University in Fairfield, CT.

In her role, Hallie collaborates with other business leaders as she advises Duly on legal, regulatory and compliance matters. Hallie joined Duly in 2020 following prior roles at Sonova Group, Jackson Lewis PC, Ungaretti and Harris LLP and Kirkland andEllis.

Paul Dumas, Chief Human Resources Officer
Paul Dumas serves as Chief Human Resources Officer of Duly Health and Care. He is aglobal, human capital business and operations leader with more than 25years’ experience in both publicly traded and privately held high-growth organizations. At Duly, Paul leads our human resources team in Duly’s continued commitment to fostering an inclusive workplace where diverse talents can flourish. He is astrategic partner with colleagues on the management team, aligning HR initiatives with business goals while ensuring employee well-being and engagement.
Prior to joining Duly, Paul served as Chief Human Resources Officer for Cision, aChicago-based, global leader in marketing and communications and previously as the Chief Human Resources Officer for Fiserv, the global leader of financial services technology and solutions. Paul has held leadership positions at U.S. Anesthesia Partners, Merck, Pepsi-Cola andHyatt.
Paul earned aMaster’s Degree in Human Resources &Organization Development from the University of San Francisco, specialized training through the Human Resources Executive Program at Stanford University, and aBachelor’s degree from Johnson &Wales University.

Donald Hoscheit, MD, Chief Medical Officer
Dr. Donald Hoscheit, apracticing board-certified gastroenterologist, serves as the Chief Medical Officer of Duly Health and Care. He has been in practice at Duly for more than 35years and has held many leadership roles within and outside the organization. In addition to serving as Chief Medical Officer, he leads many of Duly’s internal committees and shares responsibility for upholding the integrity and accountability of the staff while empowering them to best serve the members of our community who come to us forcare.
He earned his medical degree from the Loyola University Stritch School of Medicine and completed his internship, residency in internal medicine and fellowship training in clinical gastroenterology at the Loyola University Medical Center. He is afellow of both the American College of Gastroenterology (ACG) and the American Gastroenterological Association (AGA).

Chris Nash, Chief Technology Officer
Chris Nash serves as the Chief Technology Officer of Duly Health and Care, where he leads ateam of nearly 200 technologists and engineers. Prior to joining Duly Health and Care in 2022, Chris served as CIO of Adtalem Global Education where he helped transform Adtalem into aleading educator of doctors, nurses, and veterinarians. In 2019, Nash was awarded Chicago CIO of the Year for Large Enterprises. Chris has held numerous management and executive positions with avariety of technology organizations from Kraft Foods and Millward Brown to Shorr Packaging. His technology experience spans across digital transformation, eCommerce, data analytics, AI, cloud migration, andmore.
Chris earned his MBA from Northwestern University and his Bachelor’s degree from Beloit College.

Charleen Recto Phillips, Chief Accounting Officer
Charleen Recto Phillips serves as Chief Accounting Officer at Duly Health and Care. In this role, Char is responsible for our accounting and financial controls framework. Char has more than 17years of progressive accounting experience. She is focused on leading and integrating the accounting, financial reporting and system implementations teams to ensure support for continued growth. Char worked in various private equity-backed companies in different industries including healthcare and technology. Most recently, Char held the role of SVP, Accounting at Healthcomp where she managed the corporate accounting, accounts payable, accounts receivable and accounting operations functions for the private equity backed Third Party Administrator. Prior to Healthcomp, she served as VP, Controller at Milwaukee based Advicent (now part of InvestCloud) and VP and Controller at Centro (now Basis Technologies).
Char holds abachelor’s degree with dual major in Accountancy and Management from DePaul University. She received her Certification in Public Accounting in Illinois in2015.

Dana Rye, Chief Value Based Care Officer
Dana Rye serves as Chief Value Based Care Officer. She leads our value-based care strategy and program management, including analytics, care management and population health management. She joins Duly Health and Care from US Renal Care, anational provider of kidney and dialysis services. She served as Senior Vice President, Value Based Care Operations. She led multiple value based care programs, responsible for strategy, operational execution, and physician relationships. She stood up the company’s first value-based care program nationally, generating successful outcomes in nearly all markets. Finally, she built and led the Transplant Operations team, responsible for supporting patients navigating the transplant process. She began her USRC tenure supporting growth and operations for home therapies.
Prior to US Renal Care, Dana held Business Development and Operations roles at OneOncology, an MSO for community oncologists, helping to build the de novo organization and physician network. She joined the private-equity backed healthcare services space from McKinsey &Company, where she worked in the Healthcare Systems &Services practice. She began her career at Credit Suisse, most recently serving as aVice President in the High Yield group covering healthcare companies.
Dana holds an MBA from Harvard Business School, an MPP from Harvard’s Kennedy School of Government, and aBA from Middlebury College. Dana serves on the board of Fight Colorectal Cancer.

Rajit Shetty, MD, Senior Vice President, Clinical Operations
Dr. Rajit Shetty, board-certified in radiology, serves as Senior Vice President, Clinical Operations at Duly Health and Care. Dr. Shetty works closely with the Duly leadership team to lead improvements in our clinical performance.
In practice at Duly Health and Care since YEAR, he most recently served in aphysician leadership role as Division Chief over Ancillary and Medical Specialty Services. As apracticing physician he has clinical interest in neuroimaging, specifically involving the brain, spine and nervous system. He has been instrumental in advancing radiology services at Duly and building adepartment of specialists with advanced training in all modalities of imaging.
Dr. Shetty earned his medical degree from the University of Kentucky College of Medicine. He completed his residency training in radiology at St. Louis University Hospital and his fellowship training at Brigham and Women’s Hospital Harvard Medical School.

Paul Travers, Interim Chief Financial Officer
Paul Travers serves as the Interim Chief Financial Officer at Duly Health and Care. At Duly, Paul is responsible for strategic and financial planning, cash management, accounting, budgeting and financial reporting. He brings nearly 30years of financial and business development experience across various businesses to his role at Duly. He joined Duly in 2019 as Vice President, Financial Planning and Analysis. Prior to joining Duly he served as Vice President of Finance at Allscripts; he also worked at Deloitte and ValueLine.
Paul received abachelor’s degree in finance from Fairfield University before earning his MBA in Finance and Accounting from The University of Chicago Booth School of Business.
